Dr. Iraniha Laparoscopic management of gallstone ileus
This is a 76 years old female patient who presented to the emergency room with severe abdominal pain, nausea and vomiting and her ct scan showed small bowel obstruction due to a large gallstone ileus that stuck in the small bowel. This rare condition is called gallstone ileus. This problem usually happens in patient with very large gallstone that causes chronic inflammation in the gallbladder and adjacent small bowel (duodenum), creating an abnormal connection between these two structures called fistula.
Eventually the gallstone falls into the small intestine and migrates distally and causes small bowel obstruction. she underwent a diagnostic laparoscopy surgery and the stone was removed from the small bowel to relieve the obstruction. This operation was performed through 3 small, one cm incisions and took just 50 minutes.
This surgery decreased the patient’s postoperative pain, improved her recovery and eliminated the requirement for resection of her bowel or large open incision. Minimally invasive surgery including laparoscopic and robotic surgery really improves the patients’ outcome and makes an amazing difference in their recovery
