Extremely difficult gallbladder surgery
This is a 45 years old female with five years history of episodic upper abdominal pain whose ultrasound showed gallstones with thickened gallbladder wall , consistent with chronic cholecystitis. Initial inspection with laparoscopic technique showed chronic cholecystitis with significant thick, firm adhesion and scar tissue around the gallbladder, making the minimally invasive approach with conventional laparoscopic equipment almost impossible. Normally a conversion to open technique is the only option in these situations. This video demonstrates the application of the robotic platform to perform an extremely difficult gallbladder surgery with minimally invasive approach and avoidance of conversion to the open cholecystectomy. The wrested instrumentation and superb visualization of the robotic platform allowed the surgeon to proceed with minimally invasive approach to remove the gallbladder. This patient tolerated the procedure very well and was discharged home on the same day. Her drain was removed in one week after the surgery and she went back to work and resumed her regular exercise and activity in 2 weeks.
